I'm in need of a good check list that I can use to evaluate my ADD symptoms. My pdoc wants me to report on how my symptoms are doing while on medication. Maybe something that has a scale or a way of noting intensity. We are trying to evaluate how well the medication is doing at different doses and if there are any problematic symptoms that the medication isn't touching.

Any ideas on where to find something like that?
