Have you tried any tools like myfitnesspal? It's a really great tool to help you keep track of your food intake and workouts.
Just try and be consistent, it's more about a lifestyle change than trying to lose weight quickly. That's the only way to maintain weight loss. Find things that you like doing, like swimming or playing tennis so that you don't feel like you are working out. I personally like getting outside to do my workouts (when it's not too hot) and alternating between biking, swimming and walking so I don't get bored with it.
