My son is on the following:

1mg of Klonopin twice a day
100mg of Zoloft (we may increase it again but we're staying at 100mg for now)
.25mg once a day of risperidone, which may be increased to twice a day next week.

He was on klonopin for a few years (because he was afraid of antidepressants due to a bad experience of increasing too quickly and getting suicidal thoughts when he was younger.) But the racing and intrusive thoughts became so unbearable that he broke down and started taking Zoloft a few months ago. We increased VERY SLOWLY via 12mg per week and only got to 100mg a couple weeks ago. We decided to hold steady at 100mg for now. The Zoloft made a tremendous difference. It has almost completely eliminated his racing thoughts. His intrusive thoughts are only once a day and he doesn't OCD about them for more than 15 minutes; whereas previously he would OCD about them for days and days.

The risperidone was added because the Zoloft didn't help with the paranoia. It's subsiding. He has only been on it for 5 days. Fingers crossed that it works.
