It's important to be honest and open with your providers. It's the only way they can help you.
As for what to do, it may be time to get off the gerbil's wheel for a bit. Trying to continue to push through it may ultimately cause you problems at work, either with your performance or by possibly blowing up at the wrong person at the wrong time. There are intensive outpatient or "partial hospital" programs that give you more focused treatment while you can still go home in the evenings. If you're in the US, you work for a company with more than 50 people, and have been with your employer long enough, you are covered under the FMLA and can take the time off you need to treat your illness.
I wouldn't wait much longer. It could save not only your job, but your life.
