Practically speaking, depression doesn't care about reasons, but you already know that too well.
A downward spiral is something your T needs to know. Have you or can you tell your T about the visible signs of this spiral, the symptoms such as excessive smoking, unstable appetite and insomnia that can be seen? Can you talk in general about "very low mood?" (Personally, I regard insomnia as a Big Deal.)
Ideally you should be able to freely share with your T without fear of overreaction. Do you have reason to believe your T would overreact? Do you have the emotional capability to be more honest with your T?
No need to answer these questions; they are just for you to consider.
