I doubt that every patient is aware of their treatment plan, as it's mainly the creation of the therapist to help keep the patient on track solving the presenting problem.

People who have been hospitalized are often brought in on the planning process as a means to engage them and show them hope for future...and to help them stay on track after discharge.

Certainly though, every good therapist has some kind of a treatment plan for each patient, once they are sure where the patient wants to go.
