I can really relate to not being able to sleep at night, and then not being able to wake up in the morning. It's hard.

I find that it helps if I set multiple alarms so that they go off every ten minutes. Usually after about an hour of this, I can get up.
Also, try to reschedule your pdoc appt soon. It sounds like you could really need some help with your depression.
It feels like you can't handle it now, but you're stronger than you're giving yourself credit for.
