things may have changed since i applied 22 yrs ago, but i was denied the first time, and i gave them all the medical records from off of my pill bottles~! :O

if you have to appeal, DO be prompt,, they mean it when they say 30 days~!
i was told to use the reason for appeal as: Condition has Worsened.

any additional records you need to gather to support that reason can be sent later. it's the Appeal Form that must get back to them on time~
