I'm very lucky. My T has a private practice, but he has a kind of contract with the public health system so he's paid by my tax kronor. As a result I only have to pay what I would for a specialist physician - a fairly small fee, up to a maximum of approximately US$ 170 per year. After that I don't pay anything (for any kind of health care) for the next 12 months. I didn't think it would be possible to get long-term therapy covered by the system, but evidently it is - so I really am fortunate. I would not have been able to get this level of care if I'd had to pay out of pocket. The therapy is better, too, when I'm not feeling anxious about wasting money.
