Yah... what LMo said... lots of time it's the insurance who is like that's not on our list of "approved drugs" and they won't cover it unless they get a letter from the Dr. telling them why you need it.
I had an issue like this once... it was for a yeast infection and the dr. perscribed one kind of med but the insurance company wouldn't cover it so the pharmacist filled it with the "approved" perscription. So... 3 weeks later when I STILL was having issues I had to go back to the dr. AGAIN turns out I STILL had a yeast infection. Apparently I had 3 different types of bacteria and that medicine only worked to kill ONE! Let me tell you I was in major pain/ discomfort for OVER a month... I'm still fighting the insurance company over that.
After many many issues with my health insurance I've decided I HATE INSURANCE companies!!!! I feel your frustration.
