I have both. I googled Advance Directive Minnesota and a form was found. I filled it out, both the psychiatric and medical one. Then I signed with witnesses at a notary. (Most banks will do it without charge if you have an account with them). I gave a copy to the hospital in town, my medical clinic which includes my psychiatrist and then send copies to the two people I named as agents. I kept the original copy.

My brother-in-law Jack died without one in place. He was on a vent, tube feeding, ICU for almost a month. I have to wonder what he would have written in an advance directive.
