Ahdm; it's clear you need far more than CBT to help you. This is the NHS at it's best ( #worst) giving inappropriate therapy in the hopes it makes you go away, CBT is cheap and quick and that's why they dole it out. My advice is write a letter explaining the issues that still remain for you ie self harm, abandonment issues etc etc explain that you need further more in depth treatment and give the letter to your Dr or write to your CAAMHS team.

Also, look around your area for charities that might offer free counselling, if you're at college they should have a counselling services. Some private therapists will offer a sliding scale too so work out what you could afford and ask them.
