Call your US congressperson's office and ask for help in completing the paperwork. One of the administrative assistants should be very familiar with what's necessary. If they can't, call your state rep's office.

These are public employees, and some one of them has the knowledge you need. If all else fails, appeal to the reference librarian at your public library.
