I was not sure how to vote because the thread specified in session notes while the question just asks about notes in general. My therapist takes notes after each session but only very occasionally writes anything down during session. He shares his notes with me from time to time. They are not terribly interesting, just very general comments about my mood and general references to what we discussed with no detail.

So yes, he takes notes after each and evry session, but only very occasionally during session. Doesn't bother me at all when he does by the way.
