I think pretty much every T I have had took notes. One I had would use little yellow stickie notes.

My previous T took notes, and at the last session I found out she printed out EVERY email I'd sent her and put it in my file. Yikes!!

My current T takes notes in each session, but they don't interfere with things. She keeps a notebook/tablet just with my session notes - because she will refer back to things sometimes by flipping back. Sometimes I think about asking to see her notes. This T is really cool though because she told me if I wanted to request records from my previous psychiatrists and hospitalizations, I could have them sent to her office and she would go over them with me.
