If you have a new one they could do it for you and if not maybe your therapist can write you one if possible. My therapist wrote mine since she knows me better than my psychiatrist since I see her more often.

ive been in and out of college for 5 years. i turned in the initial paperwork 3 years ago. i had 1 month to do it. and i hounded my psychiatrist about it for that whole month. he never faxed them or gave me the paperwork or nothing. the schools policy is if you go pass the deadline you cant get accomodations forever. it was the worst psychiatrist i ever had. i dropped him and got a new one after that. i begged them to allow me to resubmit an accomodation letter but when i got a new psychiatrist but they they said their policy is a forever policy.
