Name your symptoms, stress the anxiety part of it and say that you're not supposed to feel like this anymore, so please anything different than what we already tried? I'd think it would be wise to also tell her if she is unable to help you it is in your best interest to get referred to a specialist. If you vaguely hint Enough is enough, she might actually help you if she doesn't want to lose a patient.
GP's don't normally prescribe topamax?
