I don't know much about Kaiser, but that was really unprofessional of that doctor. If you remembered her name, maybe you could talk to one of the administrators and explain what happened with your previous hospitalizations and ask to not be seen by her again. I'm not sure if that'll work, but its worth a shot.

Personally, if it came down to those three choices, I would go through with the treatment recommendation and after you show the doctor that you are eating healthily, maybe he/she will begin to trust you more?
