I can understand your concern; I had it too, really worried in fact. However, it was unfounded; my review consisted of a simple form letter that was pretty short and included some such questions as:
Check boxes as to level of functioning/improvement: worse, same or better in last 6 or 9 mos; check one box whether I had gone back to work, attempted it, talked over with doc, or not considered it; with some additional boxes here and there that needed to be checked about if might have attended school during that time and level completed; list last 4-6 (I cannot remember, but it was at least 4) dates of visits to any medical providers and the reasons for the visits (I just called each of my providers to get the dates and knew what they were for, or can ask them if need be to get the info).
I wish I could remember the rest off the top of my head but can get out my copy if want more information. I understand sometimes there will be a phone interview for a similar form.
