Two photo copied sheets. One is "Worksheet 10.1: Mind Over Mood Depression Inventory" Circle one number for each item that best describes how much you have experienced each symptom over the last week. (from not at all to most of the time)
The other one is from The Anxiety and Phobia Workbook. A check list of 'human needs'.

I would have responded sooner but I had to wait for the "5 posts in 24 hr period" to pass ...

I guess I'll stay away from the games threads for now to save my posting allotment... LOL
