I went through the permission issue because I had surgery 5 days after my 18th birthday. In terms of the wavers I had to sign before having the surgery, I had to be 18. So for things like that, I'd imagine you'd need a parent. Heck, we have to sign an xray waver every time we get xrays and if under 18 a parent has to sign.
