I know several people who take Topamax who describe the same thing. They lose track of whether they dreamed something, thought it or actually did it. Next time you see your doctor, it might be a good idea to discuss it and see if you would benefit from a change in meds or dosage. I don't know if it's caused by your particular meds, but it can definitely be a side effect for some people. Not just psychiatric drugs either. It can happen with antihistamines, pain killers, some statins and the drugs they give to stop incontinence.
