Not sure if it's part of an ethics code, but it is recommended practice that all Ts have a "professional will" which includes a designated colleague to notify clients in the event of a T's incapacity or death. It also involves instructions about the disposition of written records and other practical matters. It is written in concert with the legal requirements for the maintenance of records, which in many states is 10 years.
