It can be both asking questions and taking tests. It depends on what the doc thinks might be wrong. For example, if the doc thinks you are depressed they are probably going to talk to you and take a detailed history of what's going on. They might or might not give you a written test for depression. Conversely, if the doc believes there is something like dementia going on they are likely to ordered tests, both written and medical, to help with the diagnosis.
