2 things happen; etoh in any form is a broad hepatic inducer, so drinking while on meds will make your body metabolize the med much quicker and sometimes with poor outcomes. Also, etoh acts via increased dopamine on the pleasure centers. If you are on something that even mi nimally inhibits reuptake of dopamine you can get agitated, shaky etc...
