What a mess, I'm sorry you're going through this. Based on what's happened before I can't envision this scenario playing out in your favor. If you can manage it I would seek out another provider, possibly one that feels less like a corporation (his practice seems to have far too many patients and far too many people to go through to access help.)

Kudos for sticking up for yourself and pushing to get the attention you need. I could never do that, I'm too much of a pushover.
