Sorry you're feeling so bad. Paranoia is a terrible thing to have to live with. It definitely sounds like you either need a different AP or serious dosage increase. I know that for me the paranoia faded off into the background once I was on powerful enough meds. I hope the NP is able to bring you some relief at your appointment.
