I too have DID and I would suggest that you take another adult with you that knows some thing about your condition (or) have a one to one talk with your protector and explain to him/her that you need to go to this appointment to make sure that the host body that every one lives in is well and healthy..... maybe the protector will keep them all safe and give them some thing to do while you are in the doctor office and up on the table.

.... What do YOU think of that? - might one of those ideals work.
