I think these two ideas are good. I would suggest having things ready when she comes to get you, so that way she know's you are not trying to waste time and she needs to get on it. Maybe you guys can also start talking when she is taking the money? Also, I remember in my contract it specifically stated that each session was for a total of 50 minutes, to include the time of taking payment and scheduling next appt. So, while my T doesn't stick to this, it may be something 'already disclosed' in writing.
