So today I filled out release forms for one hospital and my x pdoc to send hospital admission/discharge dates, meds and dosage( both hospital and pdoc), and ALL DX's(both hospital and pdoc).

It was just so creepy to go into those places and be reminded of how bad off I was once was. Talk about being extremely grateful! I couldn't be more so...for how far I have come. I can't even believe it...sometimes I pinch myself to make sure I'm not dreaming.
