I've had the Mirena for the last 7 years. I'm on my second one. I've never been pregnant, let alone carried a child to term. They make different sizes of the IUD to accommodate whether you've given birth or not. Your partner will not be able to knock the IUD out of place regardless of how big he is. A properly fitted IUD will not go anywhere until the doctor takes it out. The string isn't so long that you'll know it's there unless you go digging around for it. It is good to check to be sure that it's there every so often, lol.
My migraines went away and my mood improved drastically once I got the Mirena. It does hurt going in, though. Your doctor can inject an anesthetic into the cervix. My doctor doesn't, because she says that the injection hurts almost as much as the insertion. She thinks it's better to get it done and over with. Less anxiety to go through. The second insertion was much easier for me. Other Mirena users have said that their third insertions weren't bad at all. Besides, you would go through this only once every 5 years.
I encourage you to get the Mirena. It really knocked out the PMDD for me. I was sick with PMDD for 3 weeks out of 4 until I got the Mirena.
