I've been on SSDI since 2004 and I was reviewed in 2010. It's just a questionnaire and you fill it out with your Dr. At the end it asks for any additional comments and you can write about your treatment, personal history, etc. yourself in detail and explain problems you are still dealing with. If you haven't had any improvement don't worry about the review - you'll continue getting SSDI. I heard from SSA within a month after sending the form back to them and they had no problem continuing my SSDI.
