This is extremely shocking - to be discharged from all services even after a recent hospital stay is downright appalling, not to mention a breach of care guidelines. Aftercare is your right after hospital treatment - which effectively means they're not allowed to discharge you unless at your request. You need to speak to the patient advice and liaison service immediately and explain this terrible situation to them. They should be able to sort things out for you and if not you need to get in touch with the top boss of your CMHT's local head office. There's also MIND who can advise on the appropriate legal/advocacy options. Who's your care coordinator? Did they have no input at all? Failing all of this, you might need to go to your GP and see if they can rush through a re-referral to your old doctor/team.
You should have the same consultant in charge of your care at each review and a care coordinator (be it cpn, social worker, or therapist) to help liaise appointments, referrals and handle overall emotional wellbeing.
It sounds like you've been atrociously let down - mental health services are bad in this country but not normally this bad. Please know that it's not supposed to be like this and that the appropriate care is out there if you know what to do and who to speak too.
