Off to my right at a blank wall. Once I looked up and just past the therapist's head out the window and it drove her nuts. She kept asking what was out there and turned around to look herself a few times. That was sort of fun. The office is a couple of floors up and all one can see is some tree branches.
I only look directly at her when I ask a question or if I am angry.
