Have some ideas...
Work with a nutricianalist to develop a food plan that works for you and helps you stay away from triggering foods
Not sure how big your family may be - keep triggering foods out of the house
Do not shop for your food if you feel this is triggering
You may also want to consider talking to someone professionally on a regular basis (you mentioned rehab, but not therapy) what you see in spandex may not be what other people see...
I'm guessing some of these things are not really new to you...
