Yep.. it is in the paper work that I filled out at my initial appointment. It was something I was reminded of recently as I was filling out the paper work for my son to be seen at the same practice.

It is done now..he didn't do anything wrong ethics wise. Just thought he could of handled it better with me. Still stewing and still thinking about emailing him.
