I am anxious so much that I try to get out of doing these things, which I know I need to do.

I like sideblinded's book recommendations too.
I used to have exactly the same thing. You might want to have a look at this and see if it describes why you have anxiety about upcoming new things. There is a very easy thing to try ("SNAP CLUB") which has been a huge help for me and others as well.
