He does a quick check with me at the beginning of each session and makes note of how I self-report my mood, etc. Other than that, the only note-taking he does during session is if we get on the subject of meds and he makes note of those. After session he makes notes about the session. They aren't extensive; they're pretty general and clinical (he's read them to me on occasion when we were trying to remember something from a previous session.) Nothing earth-shattering.
