Just remember that YOU are hiring THEM to provide a service, and you don't owe them any more info. than you are comfortable providing. I would go with very, very short answers where "necessary," and it's definitely ok to leave some blank (no matter what they ask for). It's also ok to say you don't feel comfortable discussing some of it if they ask for more info. during your first appt.!
