I pay cash and didn't fill out any forms. I just wrote down my name when I did a test.
In my country we have universal health care. From what I've read, it's really bad when it comes to mental health. You first have to go to a doctor, he/she will send you to a neurologist which will check you for any neurological diseases and what not. When they realize there is nothing physically wrong with you they send you to a psychologist. A psychologist will listen to you for 20 minutes and then prescribe you drugs.
