I have found that the most effect approach with academic administration is to be open and honest. If you have personal stuff going on right now, I would definitely recommend going to the counselor and explaining your situation. Believe me, they have heard it all. You might feel embarrassed but I would be very surprised if you said something that they haven't already heard from someone else. I know it probably feels like you are all alone though.
When you meet with them, it's important that you be clear about your desire to graduate and raise your grades. Professors look for the student to take initiative. If you go in with an attitude that says, "I'm willing to put the work in, and I need some guidance as to how to achieve X,Y,Z" , they are going to be a lot more understanding and flexible to work with you. If you plan on taking summer school, I would mention that. Make it known that you care about your academics and that you are trying to make a plan. That proactivity will go a long way. Lastly, I suggest going into that meeting knowing what your goals are. The conversation between you and the counselor should be a dialogue. In order for them to best assist you, they need to know how invested you are. Try to keep an open mind and don't be afraid to ask for help.
