Jennifer, you can call the ins co back and ask for the approval number/ authorization number that they give the billing party... that way they can't say they didn't approve it
(Well, they can always say that but with the number, you have legal reason to count on it!)
Take a deep breath. Would it help to know that nearly everyone of us go through similar processes when we need extra help? It's the only way the system seems to partially work sometimes... by our constant cranking on it.
