Both should dig deeper. I was first misdiagnosed with depression and put on an antidepressant. I continued to really get worse so I changed docs. So bad were I managed to get fired from work. I told my therapist at the time and she seemed to support the diagnosis of the DO. I went to a mental health provider and signed release for them to get my progress notes from the DO and the therapist. I saw a new therapist for four weeks and was diagnosed with bipolar 1. In the process of the four weeks we went over my story, family history and my current story at that time. My prior case notes helped a great deal I was told. When I received the correct diagnosis I was shocked. Then I was pissed. I called the DO and the manager of the therapist and let them know how much they screwed up. You don't give someone with bipolar 1 an antidepressant without a mood stabilizer it flips them over into mania.
