Confidentiality rules may vary somewhat from state to state, but generally there are only 3 times a counsellor can disclose info. 1. you are at risk of harming yourself, 2. You're a danger to someone else. 3. A minor child is being abused.
If you're seeing an MD, they may have additional reporting requirements. For example in Ontario, if an MD thinks you're alcoholic, and so could be at risk of drinking and driving, they are required to report it to the Ministry of Transportation, which will then suspend your licence for up to a year. I personally don't think that's fair, but it is the law.
