Good job on reaching out for support!
In my experience, the best thing to do is stay focused on what YOU need in the therapeutic relationship. If you hold something back, or kick it down the road, it might get forgotten and never addressed.
What if you printed out what you wrote above and just take it to your therapist? Bring it in and just say, here is what I need to work on, and I need your help with all these things. It is a good summation you made, and can be taken point by point. Your care team won't find the list daunting. Believe it or not, they have dealt with it all before.
