I emailed my old pdoc to say that I would no longer be seeing him. You don't need to provide any explanations. Just set up an appointment with your new pdoc. My new provider wanted to talk to the old provider so that my records could be transferred. I OK'd that. Not sure if everyone does this. Of course, you could also explain to your new provider why you decided to make the switch. Good luck!
