I don't think it's a matter of lack of self awareness, but rather just a collaborative way of interacting. Some posters say they have these conversations naturally, but a lot of times that doesn't happen. I think it's good practice for a therapist to ask clients if they find therapy helpful or not. I wouldn't want one making assumptions based on my demeanor in session, since that isn't often a true indicator of how things are outside of session.
