I have a similar situation. One of my favorite professors who is extremely nice and thoughtful (he even donated a lot of money to my club's charity after I asked him)...we were having a conversation and he asked what my plans were for the summer and I ended up mentioning I hadn't found an internship yet. So he later emailed me with a contact who needed interns... I emailed this contact my resume, he asked me out for lunch, and I never followed up. The thought of a one-on-one encounter freaked me out and I just avoided the situation. Fast forward to after the summer, I ran into my prof again and he asked if I ever contacted the dude. Had to tell him that I didn't really follow up, which was really embarassing after all he has done for me.
