T doesn't take notes during session. Most of the time I forget that I even have a file. Previous T made a point of telling me during my first session that she doesn't take notes in session. That did change though - I guess there was so much transference that she didn't trust her memory. I certainly did not agree with her account of our earlier sessions when I saw her notes later...
