It is possible that the psychotic episode will end on its own, and that he will have windows of clarity. That's how it was for me. I had similar delusions when I was at my worst, the paranoid type and I couldn't even hide how bad it was anymore.
Honestly if I had to guess, your son already has had windows of clarity in between episodes of psychosis, and already knows during those windows that something is wrong.
But accepting treatment isn't as simple as just realizing that something is wrong. Treatment can be terrifying, humiliating and infuriating all rolled into one, for several different reasons.
Frankly, I think it's a process that everyone goes through, as far as getting to the point of accepting treatment and help, for those of us who do. There is no way to really force someone through it, it just has to run its course, the person has to get to that point on their own.
The best you can do is to be loving and supportive in the ways that you can, while remembering that ultimately it's beyond your control and not your fault.
